You must be able to maintain enough downward slope in the extension so that the water will drain properly. This isn't much for a 4' run (about 1/8" drop), so that may not be an issue. Since there is no wall going up to the ceiling for an island, it has to be done a bit differently. The best venting solution for an island is called a "Chicago loop".
